Step #2: Locate the member services/customer service number on the back of your card.
Call that number to check for benefits with your insurance company.
Step #3: Ask the representative the following questions & write down the answers
1. Does this policy have nutrition counseling/medical nutrition therapy benefits with a Registered Dietitian? Provide the following CPT codes: Medical Nutrition Therapy: 97802 (initial visit), 97803 (follow up visit), 97804 (group).
2. Is this benefit limited to a specific diagnosis or co-morbidity? If so, which ones?
3. Is preventative care covered under ICD-10 code Z71.3 Dietary Counseling & Surveillance?
4. Is my provider in-network or out-of-network with my plan?
